Supervisory Nurse (Case Management)

Provide assessment of the patient's needs and personal support systems, development of an individualized care plan, coordination of the services required to implement the plan, client monitoring, and periodic re-evaluation and adaption of the plan. Independently plans and provides nursing care based on professional nursing practices and protocols and medical protocols for patients. Handles complex referrals from other providers in conjunction with medical staff; schedules diagnostic studies, clinic appointments, medical procedures; coordinates with other service lines as required to meet patient needs. Develops with the patient/family and multidisciplinary team members for a case and service delivery plan based on needs identified and available provider and financial resources. Coordinates multidisciplinary patient care conferences for the high risk or medically complex patients. Obtains needed services for patients through arrangements with service providers. Identifies gaps in available community services and resources. Serves as a resource on issues concerning case management. Supervises two managed care nurses. The incumbent plans work to be accomplished by subordinates and prepares schedules for completion of work; assigns work to subordinates based on priorities and capabilities of employees; evaluates work performance of subordinates; advise or counsel employees on work and administrative matters; interview candidates for positions, recommend appointment, promotion, or reassignment to such positions; hear and resolve complaints from employees; effects minor disciplinary measures, such as warnings and reprimands; identify training needs of employees; and approves leave, etc. Ensures annual review and/or revision of Case Management policies and maintains appropriate documentation of these activities. As the Case Management expert assists with interpretation and ensures enforcement of their provisions. Basic Requirements for the GS-12 (or equivalent) Grade Level In addition to the mandatory license and education described above under Basic Requirements applicants must have at least one or more full years of professional nursing experience that is equivalent to the next lower grade level. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:
Experience and knowledge of Case Management and Care Coordination processes to develop policy and procedures for a case management department. Experience in the development and administration and supervision of the Case Management Department of the clinical component and appropriately involves the case managers and care coordinators. Experience in establishing, implementing, and managing a model of best practice that is appropriate for the needs of the Service Unit. Experience in implementing goals and plans to assist/support case managers and care coordinators to implement quality of care and managed care monitors and controls in health care activities. You must meet all qualification requirements within 30 days of the closing date of the announcement. Federal employees in the competitive service are also subject to the Time-In-Grade Requirements:
Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
